Book Match for Kids

Welcome to the children’s book request form!

This form is for parents who would like to borrow items for their child to read. Please complete this form for each child.

Fill out the fields below and our Children’s Services Coordinator will select at least 5 items based on their interests and place them on hold for you to pick up during library hours.

Selections may include books on cd, graphic novels, level readers, non-fiction and/or fiction. Use this form as often as you like!
